Make a video A 27 years old male patient while being evaluated for dyspepsia was found to have some cystic lesion in the right lobe of liver but couldn't get characterised on abdominal ultrasound. Contrast enhancement CT scan revealed well defined cystic lesion in segment 7 in sub diaphragmatic location. Differentials of diaphragmatic mesothelial cyst and lymphatic cyst. We decided to characterise the lesion on Endoscopic ultrasound. What we saw ultrasound was quiet remarkable. There was a large lesion of size around 4 × 4 cm in the segment 8 extending to segment 7 posteriorly. Lesion was abutting the diaphragm
